We didn't. All it is is a ratchet. You really have to pull *hard* on the belt to get it tight enough, though; if you're not happy with the way the seat is secured, it should enable you to take some extra slack out of the belt.

If your vehicle has LATCH connectors (I think 1992 is when they started becoming standard on cars), use those instead (if your seat supports them, I believe most do these days) -- it's much easier to to get a secure installation (imo).

Short answer: No. Long Answer:

The trick all parents learn is to literally kneel in the seat while you are tightening the belt, the, depending on your belt, either you are all set or you have to install a metal 'belt holder' that comes standard on all child seats.

Regardless of anything else, car seat bases are still proving to be tremendously unreliable (the kind of seat sold most often for newborns with a carrier that snaps in and out), so please, strap the seat in but leave the base at home(in a recent government test, 9/10 or some outrageously high number of bases failed to prevent injury, some severe).

You do not need it with this car seat.

I am assuming your car model is older than 2002? If so you probably don't have the latch system, but if it's 2002 or later, you have the latch system.

Call if you have any questions.

Drive by a local police department for an inspection of the installation if you have any doubts. Usually they are happy to do a safety check for you.
